King’s Daughters Walk-In Care for Schools is your partner for your child’s health.
To enroll your child in Walk-In Care for Schools services, please complete the consent form by clicking on the link below. If you would like for your child to receive recommended vaccinations at school, please complete the vaccine consent form. Once completed, you will receive a confirmation sent to the email address you provided.
Flu vaccinations will be administered in late fall. Please check back in October for flu vaccine consent.
